Order a Mass Enrollment Online
After selecting the card or folder, complete the online form and submit; we will send the enrollment out for you.  You have the option of using a credit card or waiting for an invoice to come.  You may also call the Mass Card office directly with your request.

Order Your Own Supply Online
Keep a supply on hand for your own convenience.  When you need to use a card or folder you will simply write the name of the living or deceased person in the Mass card or folder along with your name and send to the family of the deceased or to the person enrolled.  You will return your donation with the enclosed enrollment slip to Holy Family.

Order a Supply By Mail 
Print and complete the Order by Mail form and send it to Holy Family.  We will then send you the supply you requested.

All Masses are offered at Holy Family by members of the Passionist Community.  Your offering helps to support Holy Family's many ministries.
